Recommendations letter

I would like to write ( Two recommendations letter for employee for  graduate school) one page for each.
Two letters of recommendation that include:

  1.The recommender’s name, title, organization and mailing address
  2.The length of time the recommender has known the applicant
  3.The recommender’s role or capacity in their interaction with the applicant
  4.The recommender’s evaluation of the applicant’s potential to be an active, involved and successful participant in the DM program.
