IHP 670 Milestone Two Guidelines and Rubric Healthcare managers produce a program design map that consists of a logic model. This model consists of program inputs (resources), processes (activities), and outputs (outcomes). This map is used to communicate the expected outcomes and resource demands to any of your local and/or community stakeholders. The activities within the program design map also demonstrate how the flow of health services will occur within the context of the program. The successful completion of this milestone will assist you in the creation of your final project. For this milestone, you will create a program design map by developing a logic model. Be sure to include the following elements in this submission:  Identify key organizational stakeholders and discuss their relationship to your proposed program. Which members of the organization should be involved and why? What role will they play?  Create a logic model using the native tool in Microsoft Word or a similar word processing program. Consult the textbook and supporting resources located in the course to assist you in designing your logic model. Be sure to include the following elements within your logic model: o Resources or Inputs: Consider human, financial, organizational, and community resources. o Activities: What does the program do with the resources? Activities are the processes, tools, events, technology, and actions that are an intentional part of the program implementation. o Outputs: These are short-, medium-, and long-term definitions of outcome criteria.  Finally, assign appropriate connections and feedback loops for the logic model. Guidelines for Submission: While your final project will be a presentation with detailed speaker notes, all of your milestones for this project will be in paper format. For the final submission, you will convert your papers into presentation format by adding information to slides and speaker notes. Your paper for this milestone must be submitted as a 2- to 3-page Microsoft Word document with double spacing, 12-point Times New Roman font, one-inch margins, and APA formatting.
